Personal Wellness Profile (PWP)

What is the PWP?

The Personal Wellness Profile (PWP) is a health risk assessment designed to identify personal and group health needs and interests to be used in program planning and risk intervention. The results will provide you with the power to identify health risks, target risk groups, track progress and evaluate cost-benefit ratio.
The PWP provides the foundation for an effective wellness program. It takes various lifestyle and health factors into consideration including personal/family medical history, nutrition, stress, biometrics, and many more measurements of health risk. Assessment scores and lifestyle-change recommendations are based on optimal health factors rather than risk of death.

What do I get out of the PWP?

Provides a complete health and lifestyle assessment based on the most current scientific guidelines for optimal health.
Features easy-to-use 39 question survey available in paper or online version.
Provides confidential, personal reporting delivering immediate results to the participant’s desktop.
Offers extensive aggregate group reporting that helps identify patterns and trends in the health of your group which can be used to develop a targeted wellness program.
Economic impact report can be used to estimate average medical claims cost per employee and compare those costs to any goals you have set for your group.
Pre and post PWP assessments can be used before and after implementation of a wellness program to evaluate the effectiveness of the program.

For Individuals

Take the next step and click here for your own personalized PWP. Complete the questionnaire and print out the results right at your own desk. It is completely confidential! Fee for the PWP is $11 and can be paid for through PayPal or by mailing a check. After we receive your payment you will be emailed a username and password which can be used to access the PWP. Please allow two business days from the time that we receive your payment to get an email with your login information.
